Uneven wear on brake pad

Hi guys

Hope you can shed some light on this problem - one of my pads has significant wear on the top half of the pad, but the bottom half almosts looks new. The other brake pad appears to be wearing evenly. At this time both brake seals were ok.

I replaced these pads with new ones only a few months ago and the same thing is happening. However this time I did notice the seal was leaking on the side that the brake pad was wearing unevenly. Not sure if this is the cause or that the seal went because the pad got to the point of being so unevenly worn that it caused the piston to come out at an angle. As I said before, there was no leak when the pads had worn originally.

I did notice that the brake pads seem to sit at a slight angle to the brake disk but not sure what would cause this - the axle isn't bent, the brake disk or holder doesn't look bent - this is very confusing and would like to hear other peoples opinions experiences?
