Just to add to Joe's post the disk in question shwed no signs of damage around the edge, however we have not yet located 1/3rd of the disc but the kart was a 2004/5 Tony EV and we are led to believe that it was an original part.
All components have a 'working life' and it looks like this one was at the wrong end of it!
Another interesting fact is that we had 2 identical failurs of small OTK discs in 2007 on the same day. All of the debris was sent to John Ryan at the MSA (who interestingly was present on Sunday and witnessed the crash) for inspection. I understand that these failures prompted the introduction of the disc protectors we now use.
As Hootsman advised if you don't want to have a disc fail check your discs, and don't let John Ryan into the circuit!!!
